I filled it out, but I always wonder about such surveys.... I checked having gotten him from a shelter. Then checked he had significant heath problems (severe kennel cough and heartworm positive) but there was nowhere in the survey to differentiate between inherited problems or accidental issues.  A dog who was run over by a car may have significant health problems that in no way reflect on th breed....Good surveys are difficult to make and harder still to evaluate.
