Thank you so much! However, I'm not with Lake Shore, although I do work with them. I have Pet's Second Chance Inc Welsh Corgi Rescue in St. Louis. I cover Missouri (the puppymill state) and parts of Arkansas, Kansas, and Southern Illinois. I will follow the links that you provided and get the form to you. I will also apply to the Art Helping Animals. Thanks for the heads up for the organization.

Do I need to submit a Corgi photo, or do you crate your own?

I read your post to Bob. Would you consider doing a Corgi painting that we could put on eBay to help offset the rescue costs of the Corgi kids? Our costs continue to grow and we have had several Corgis with major medical issues.
