Help fenway keeps having accidents in the house!!!!!

Ok, so fenway is only 3 months old and I understand he is a little pup, however he is very good about going outside to the bathroom and lets us know when he has to go by going to the door. But, lately after bringing him in after he has gone to the bathroom he will pee or poop AGAIN inside the house. He is not having his accidents near the door and gives us no warning. He refuses to go on the pee pee pads we have set up around the house. Me and my boyfriend are lost, we have no clue what to do with him or how to treat this matter properly. Any suggestions would be SO HELPFUL!!!!!
We have no idea what to do, this is our first pup!!
Suggestions are welcome, please, please please!!!!!

Time, patience, routine. At three months old he is way to young to have good control of his eliminations or to be consistent with asking to go out. I personally have no use for potty pads and feel they make housetraining much more difficult.
Fenway needs to have a very established routine for elimination. Key times are first thing in the morning, right after meals, right after he wakes from a nap and just before bedtime. If you are both working he needs to be confined in a safe place while you are away.
Pups are going to have accidents frequently. While they will become less frequent I would expect him to be solid until he is about a year. Typically they do pretty well in the 5 - 6 month period but have a tendence to "forget" about the 9 or 10 month old time.
Four Paws Corgi Rescue has a great article about housetraining. Would be good to print out and keep handy. Most of all patience!

Here are all the previous threads on potty training. Like Sam said I don't think pee pads are helpful, I think they really slow you down in potty training.

Thanks for all of your input. Everything you said is what we have been doing, he goes out in the morning, after meals, after a nap and before bed. It's just the in between times while we are playing with him that he has his accidents. I think its because he doesn't want to stop playing and take the time out to let us know he has to go! I guess all we need now is patience and to eliminate the pee pads, since he doesnt use them anyways!
